Feedback & complaints
Feedback is welcome anytime, positive or otherwise, and can be given in conversation, in writing, or through an annual participant survey.

If something has gone wrong, you can raise it directly with your support worker or with Lauren, who manages complaints for the practice. You can also put it in writing and post it in. Once a complaint is received, you'll be kept informed as it's worked through, and told how it's being resolved.

If you'd rather not give your name, an anonymous complaints form is available, though this means a direct response isn't possible since there's no way to follow up.

Anything involving serious risk or harm is escalated immediately, including to the NDIS Commission or police where needed.
